**Set aside some time to fill this out. Don't rush it.** Before going forward, do you have 3-5 people that would like to join your group, group, and is there a curriculum that have picked out? Let us know about yourself and your interest in leading a group. All sections must be filled in to be considered for leadership. Group leaders shall be in regular communication with Pastor Christian Molina regarding group schedules, content and for accountability.

Personal Spiritual Info

Tell us briefly about your walk with the Lord. It is from this commitment to be intimate and regular with Jesus that you will impact those you lead. *We don't expect perfection, but do expect a passionate pursuit of a relationship with God.

Group Info

Stop and pray before you fill this out... think creatively and dream. Groups don't have to just happen in homes and they don't have to last for hours. They do however have to be centered on discipleship and be intentional about taking time for spiritual formation with each other. These can happen at coffee shops for an early morning breakfast before work, as a team participating in a sports league that gathers before or after the game, at the church building in the evening, at a park depending on weather, or a home for a more cozy feel.

Examples: Men, women, coed, young adults, moms, etc.

For example: Starting in September, every other Thursday at 7pm for 6 sessions.

Could include church, professional or extracurricular activities like sports or clubs

Give details about your home or the place you would like to meet with your group (coffee shop, Journey, sports facility), including how many you can accommodate, parking availability, accessibility for handicapped, availability to play media, etc.

What's next?

This is the start of the process.

We'll take a look at things and reach out to let you know we've received your info. Depending on our dialogue and your experience, we may partner you up with another leader for a little while, and some training to help you feel more confident or improve on leadership skills.

We may ask for a background check if you haven't done one recently. Part of our policies and insurance requirement to conduct background checks with volunteers. If there's something you're nervous about, let us know. Background checks are intended to protect our church family from individuals who are trying to hide charges of criminal or inappropriate behavior (not so much people with a lead foot, unless you're wanting to drive a bus for the church full of kids).

We do have a small group leadership training, that we would like you to complete

We'll keep open dialogue running and when it's time to open a group, we'll promote it and support you for the greatest success possible.

For now, start praying. Pray that you grow in leadership and recognize your inadequacies (we've all got them). Pray for the people who you will impact. Pray for clarity on next steps and timing.

A copy of your responses will be sent to your email address.
